Upper right side back pain is felt between the neck and the lower back at a point where spinal cord joins the rib cage. Various factors may lead to the onset of this condition. Some of the common ones are aging of the muscles, breaking off of the ligaments, muscle stiffness, abnormal placement of the rib joints, coming together of the cervical spine and disc, bad posture and/or impact on muscles from other related organ anomaly.

Work that demands extended hours and recurring body movement results in muscle stress. Desk work for example, for extended hours tires the muscles out and bad posture adds to the severity of the condition. The result is pain which might be in the form of continuous beating like sensation. If the joints have been dislocated or are not properly aligned even a deep breath may trigger severe pain. In addition, if any of the major organs like heart, stomach, pancreas or spleen are not functioning properly, one of the symptoms could be upper back pain which is also called as thoracic spine.
Each individual may have symptoms which are slightly different from others. In general, upper back pain could be in the form of knots or spasms in the muscles. One may feel piercing or shooting pain, caused by neck, arm or shoulder movement. In some instances, even slight touch to the affected area could result in severe pain which travels up to the shoulder blade.
Anyone suffering from this condition must seek medical advice. A complete physical examination is a must for the determination of the cause. If left untreated, the condition could worsen even further and the patient is unable to perform their daily activities at all. The doctor might suggest a treatment plan depending on the root cause. So the first essential step is to figure out what factors may have lead to the on setting of the pain. Treatment plan could include some home remedies like heat or ice packs, non prescription pain medication, muscle relaxants or stretching routines. More complex and physically demanding exercises should be avoided until the body is ready for them. The doctor may also advice a consultation with a chiropractor, who may treat the pain with specialized techniques involving application of pressure on the muscles, called as Flexion Distraction Technique.
